Job Description

Locations: Indianapolis and Fort Wayne, Indiana and its surrounding counties Status: Contracted - Full Time and Part Time Start Dates: Immediate openings Salary: Hiring at competitive wages $18.00–$25.00/hour. Population: Developmental Disabilities, Autism, Cerebral Palsy, Down syndrome, Dual Diagnosis, Physical Impairments, Hearing Impairments, Speech Impaired. Therapy Setting: Home and Community based Job Description You will be listed as a contractor receiving clients through the Medicaid Waiver System. DSP’s can provide individual and group therapy services in the client’s home or local facilities. DSP’s must have reliable transportation and willing to drive to and from clients. DSP providers are encouraged to work with their clients in the home and community settings. DSP providers are able to maintain their own schedules to meet their specific needs. You will have a chance to build a caseload that works around that schedule. As a contracted DSP provider, you will have full autonomy on therapeutic interventions and techniques used to work towards completing individual goals and objectives. Responsibilities listed below: Responsibilities Working independently managing your own caseload Activities of daily living (ADLs) and personal care needs Mobility assistance, supervision, and safety monitoring Hands'-on support, prompting, reminders, and encouragement Community participation and inclusion activities Track client’s goals and objectives to help prepare quarterly reports indicative of progress or regression Planning and providing appropriate music therapy activities and interventions Completing progress notes, due by the end of each month Attending quarterly meetings to report therapy progress with team members Qualifications Education High school diploma or higher Certifications CPR/First Aid Requirements State and county background checks Meet all Medicaid Waiver Provider requirments Reliable transportation for frequent travel to caseload Experience Recent graduates welcomed!
